Requirements built into the form:

• The new registered office must be an Oregon street address, identical to the agent's business office
• A consent checkbox confirms the incoming agent agreed to the appointment
• Mail drops, forwarding services, and virtual offices do not qualify as the registered office (ORS 63.111)
